We support GP Practices
with transformation.
We facilitate local transformation on behalf
of and for our members.
How do we this?
We do this by focusing on GP practices’ needs and supporting them with the introduction and maintenance of new ways of working to achieve high quality service delivery, as outlined in the NHS Long Term Plan.
How we support local transformation:
Shared insight. We work with our practices to understand and implement the NHSE strategic agenda outline in the NHS Long Term Plan, General Practice Forward View, 10 High Impact Actions and the new GP Contract.
One voice. We ensure West Norfolk general practice has a meaningful role in the transformation of the wider health system by sharing their voice at the Integrated Care System (ICS) level.
Partnerships. We build partnerships with other local health and social care providers on behalf of the region to enable an integrated, patient-centred system.
Innovation. We support our member practices in adopting new ways of working including new pathways and embracing digital technology as an enabler for their patients and staff.
Regional re-investment. As a shareholder organisation, we reinvest any surplus funds into the health system, developing new initiatives through our practices to improve patient health outcomes.
Development. We proactively source opportunities from across the healthcare system to feed into our Primary Care Networks (PCNs) and into West Norfolk.
